• A request letter for the service on Company letter head with the seal of the company stamped on the Company letterhead. The request letter should be duly signed by one of the Directors of the Company.
• All commercial/company documents should be duly legalized by the Notary Public and Consular Division, Ministry of Foreign Affairs, Wisma Putra.
• The commercial/ company documents should be submitted along with the self-attested copy of the valid passport page / IC copy (or duly notarized copies of the passport / IC) and one of the Directors of the Company.

Note: All applicants should identify themselves by presenting the original passport. (In case of submission by a company, a request letter on company letter head and duly signed by the authorized signatory of the firm should be attached).

All Malay documents are required to be translated in to English and then certified by the Consular Division, Ministry of Foreign Affairs Putrajaya.
