Life Certificate

  • Duly Completed EAP2 FORM - Click here
  • One recent photograph size ( 5 cm x 5 cm)
  • Original passport and its photocopy (1st, last and valid visa)
  • Original Pension order /PPO number and its photocopy
  • Life Certificate should be submitted at High Commission of India
    Timing: Monday to Friday (9:30 am to 12:00 pm) except public holidays.
  • The applicant needs to present in front of consular officer.
